Considering Washington's misguided activities in the years just before his death, when he came dangerously close to advocating the use of a standing army to suppress the dissent of Jefferson and his followers, it's probable good for Washington's reputation that he died when he did.

Hoo boy, that would open a can of worms. But he would also theoretically outlive his two successors (Adams and Jefferson), since they died in 1826.

Assuming he is in reasonably good health for much of that "extra period," that's going to really alter American democracy. He presumably won't retire after two terms in TTL (since he did so due to failing health in OTL), so how much longer will he remain President? If he's President for say, 16 or 20 years, that sets a completely different precedent than OTL. We are now talking about a situation where a single man can govern the country for a generation, and surely the power of the presidency will only be greater, possibly moving towards dictatorial levels...
