With a POD no earlier than 1745 have Iran become a Great Power by 2024 (have to be around level of Germany, France, Japan, etc)

Or Iran gaining Southern Mesopotamia in WW1 as it joined and because GB knows they are their puppet and then avoiding the 1979 revolution

As @EasternRomanEmpire said, it's difficult to see Iran as a great power in 2024 without giving them a Japan level overhaul sometime in the 1800s. But if the right faction takes over in 1979 they could become UAE level uber-wealthy with the petrodollars + have the massive regional influence the Islamic Republic has in OTL.
The OP said "similar to Germany, Japan..." so I think he didn't want it to be a petrol state.
Maybe Nader Shah appoints Nasrollah as his successor, not killing him and dying as soon as possible after securing his power is Mashhad. Nasrollah was a great general and could possibly have been as great as his father in military matters, defeating both the Mughals and the Ottomans; he could probably have laid siege to Baghdad and take it in 1745 if his father's health wasn't in decline; this success would have made him easily popular. The problem with this is that it just depends on how competent would the following Afsharid Shahs be and what polices would they have, extreme conservatism or religious fanaticism could have Iran as a big empire, but still backwards; but assuming that they follow a similar religious policy as Nader Shah and a similar domestic policy to that of also Nader is his best moments, then Iran could be a great power.
In case you just want to make easier you could add that Nader somehow is in a mental state that is good enough to follow the war with the Ottomans, take Kars and Baghdad. That added to a sufficiently smooth succession secures Iran as the great power in the middle for Nasrollah's lifetime.
It would really be interesting to see how the world could develop if Iran remains a great power and maybe holds Mesopotamia. It would be interesting to see them using Napoleon's invasion of Egypt and the Levant in their favour or constantly threatening and funding Indian rebels against British rules; any British enemy could have an ally that could actually march on Delhi. Russia is another important threat to Iranian power.
